Dear all,

I have received from one of our supplier ANDOR Technology - OXFORD Instruments, the following request in order to proceed. Can you please send me the requested document asap. It is a pre-requisite before requesting an export licence from the UK authorities. This agreement is necessary before ANDOR production is validated. On my side, I will have the same request to do later on with respect to French authorities. To do so, i will need this 1st document sent by you.

Therefore, please find attached the template to be used for the EUU for the ANDOR DH334T-18F-E3 Intensified CCD camera. Can you please ensure that this document is completed on the end-users headed paper NRCN and that each individual page is signed and dated by the end-user at the bottom (even if there is already a space to sign and date the page eg: page 3).

On each page, can you please ensure that the following sections are completed:

Page 1:

•             Name and address of NRCN end user (to match that stated on the headed paper)

•             Signed by the same signatory that will complete the rest of the document

Page 2:

•             Sections C and D – please ensure these details are correct

•             Sections E, F,G and H

•             Section 2 a and b

Page 3:

•             Description of the end use of the goods (should avoid military usage)

•             Signature of the official end user, date, printed name and role. If you have a web site to mention, it would help.
