Please complete the details as specified below as soon as possible.

Dear Participant,

 

Further to your order of AQUACHECK Water Chemistry PT-AQ-17C for the determination of Metals in Wastewater due for despatch on 14^th March, this part is subject to UK export control and as such, we require a special licence to ship.  Please find attached the End User Undertaking (EUU) Form.  We have already completed the majority of details on your behalf.

 

We would be grateful if you could follow the below instructions and send us back a copy of the completed form and cover letter, printed on your company letterhead, as soon as possible:

 

EUU form

1. On page 4, check that all details are correct and amend where required.
2. On page 5 (Section 3 End Use of the goods), please complete the specific purposes for which the goods are to be used by the end user. 
3. On page 5 (Section 4 End User Undertaking), please complete the relevant fields (signature, date, name and role)
4. Please print pages 4 and 5 on your company letterhead.

   

Cover letter

1. Please copy and paste the text on page 6 on to your company letterhead to act as a cover letter and complete the relevant sections indicated in brackets.

 

Please send a copy of the completed EUU form and cover letter, printed on your company letterhead, back to me via email, as soon as possible.
